When applying for licensing, arrange for final official transcripts to be submitted to the National Board Certification in Occupational Therapy directly from your school. The transcript must include the date your OT degree was conferred. The official final transcript must be sent directly from the registrar鈥檚 office, or a secure document transcript service or clearinghouse. A mailed transcript must be placed in an envelope that either has a return address for the registrar鈥檚 office OR is marked with a registrar鈥檚 stamp, the registrar鈥檚 initials, or 鈥淭ranscripts Enclosed鈥 on the back flap. Transcripts can be requested through the Office of the Registrar.
